Adult Community Transition

Adult Community Transition (ACT) was formed out of recognition of the void often created when young adults with developmental disabilities graduate from high school. Leaving the comfort of social and academic structure was more than a difficult transition—it was the loss of belonging, relating, socializing, learning and contributing. ACT fills that void by helping young adults find nurturing and supportive resources in their communities.

For ten years, ACT staff has developed relationships with North Shore businesses and organizations to seek opportunities for these young adults to learn, work, socialize and play. They include:

  • Learn and Work
    • Volunteer activities
    • Employment skills and connections
    • Life skill training
    • Educational opportunities
    • Art therapy
  • Socialize and Play
    • Exercise
    • Meal planning and preparation
    • Exploring local culture and cuisine
    • Recreational and leisure activities
